The plain film shows bilateral hilar enlargement and a bulge on the left heart border. A contrast enhanced CT reveals widespread mediastinal and hilar lymphadenopathy, which on clinical grounds was likely to be due to sarcoidosis, and which settled on steroid therapy. But what is the enhancing structure marked with a star?
