This unusual but striking appearance of a tumour infiltating the right ventricular outflow tract and causing some stenosis of it is highly suggestive of cardiac lymphoma. In a transplanted heart it is virtually diagnostic of post transplant lymphoproliferative disease. In this case, the tumour has spread around the left coronary arterial system, but has not compressed it.
