There is activity as expected in the lungs but also in the kidneys, suggesting a right to left shunt. However, there is no activity in the brain. The shunt must therefore be beyond the brachiocephalic branches of the aorta, and this can only be a patent ductus arteriosus which has developed the Eisenmenger situation.
