There were no emboli. There was diffuse mediastinal and hilar lymphadonopathy. His tachycardia was difficult to manage. Sarcoid and TB had now entered the differential, and lymph node biopsy revealed sarcoid. His arrhythmia therefore was likely due to cardiac sarcoid, and he improved rapidly with steroids.
