This is a 47 yr old Pakistani man, previously well, developed general malaise
and night sweats for a few months. Shortly after a trip back to Pakistan he
collapsed into cardiogenic shock with a ventricular tachycardia. Resuscitated,
he had poor LV function and his CXR showed pulmonary oedema. Do you think the
hila look bulky, or is it just the oedema? Coronary angio were normal and the
LV angio revealed global, inco-ordinate hypokinesis.
Partly to rule out complicating pulmonary embolism, he had a CT of the chest
