This is a false aneurysm of the button of the left coronary reimplantation. The left coronary (yellow arrow) has blown off and is separated from the aortic root (blue arrow) by the aneurysm (purple arrow). The aneurysm is clotted. Higher up, the vein graft is seen (green arrow): without it this episode would have been fatal.
