When you click here you will see an echo of a teenager who seems to have biventricular apical non-compaction. The non-compacted areas show reduced function. However,look closer and you will notice that the basal areas of both ventricles are akinetic. He was in worsening heart failure; the onset of symptoms was well before age ten. I have seen similar appearances a couple of times before, but both were infants in severe failure.

This odd combination is also well seen on LV and RV angios when you click here . The RV angio looks a bit like Ebstein's, but there was no evidence of it on the echo.

Basal akinesia is not at all typical of non-compaction and I am not sure if this case is a coincidence of the apical and basal changes or if this is a distinct cardiomyopathy with apical changes that mimic non-compaction.
